A: Your Health Coordinates are available in your Crossover account. You will see a preview on the dashboard page and more details once you navigate to the Health Coordinates page.


Q: What is the purpose of Health Coordinates?

A: The goal is to help you better understand your health, identify opportunities for prevention and follow-through, and support more informed conversations with your Care Team.


Q: Are Health Coordinates a medical diagnosis?

A: No. Health Coordinates are intended to support awareness and help you gain a better understanding of your overall health. It does not replace clinical judgment, diagnosis, or provider conversations.


Q: What kinds of information shape the Health Coordinates?

A: Inputs may include biometrics, body composition, fitness and functional measures, labs, lifestyle-related screeners, and certain chronic conditions.


Q: Do Health Coordinates replace conversations with providers?

A: No. The experience is intended to complement and support provider conversations, not replace clinical care.


Q: What happens if information is missing?

A: If required information is incomplete, you may see a partial or incomplete state and receive guidance on how to complete the remaining inputs.


Q: What will I see in my Health Coordinates?

A: You will see an overall result, areas that are on track or may need support, incomplete information, and prompts for next steps and Care Team follow-up.


Q: How do Health Coordinates account for gender identity?

A: Some Health Coordinates measures use reference ranges based on sex assigned at birth. These ranges help interpret certain health data but do not reflect gender identity. Your Care Team looks at your health as a whole person and can help explain your results in the context of your individual medical history, anatomy, and care needs.


Q: Do the Health Coordinates tell me exactly what to do next?

A: It provides guidance and prompts toward next steps, such as follow-up care or connecting with the Care Team, while reinforcing provider partnership and clinical support.


Q: Are Health Coordinates only focused on what is wrong?

A: No. The experience is designed to highlight both strengths and opportunities for improvement.


Q: How do Health Coordinates connect to the Annual Wellness Exam?

A: The Annual Wellness Exam establishes the foundational data and health baseline that powers the member’s initial Health Coordinates result and experience.


Q: Will my Health Coordinates be available immediately after my Annual Wellness Exam?

A: Not always. If all the required information has been collected during your exam, your Health Coordinates should be ready within 24 hours. If some information is still missing, you may see only partial results—or no results yet. In that case, additional data or follow-up may be needed before your full Health Coordinates can be completed.


Q: How often are the Health Coordinates updated?

A: Health Coordinates are designed to reflect your current health status. As new information becomes available, the results are updated. In most cases, information older than 12 months is not used in the calculation.


Q: What should I do if I have questions about my results?

A: If you have questions about your Health Coordinates result or what they mean for your health, we encourage you to connect with your provider or Care Team to review them and discuss any appropriate next steps.


Q: What if I disagree with or question the results?

A: Health Coordinates are intended to support conversations and engagement, not replace them. If you believe information may be incomplete or inaccurate, or if you have concerns about your results, please discuss them with your provider or Care Team.

Q: Who can see my Health Coordinates?

A: You and authorized members of your Care Team can view your Health Coordinates.



Q: Will my employer have access to my results?

A: No. Employers will not receive individual employee health information or individual Health Coordinates results.



Q: How is my data protected?

A: Member data is protected through Crossover’s existing privacy, security, and compliance practices.



Q: Are Health Coordinates HIPAA compliant?

A: Yes. The experience is designed to operate within applicable healthcare privacy and security requirements.
