On-call service to triage patients and coordinate care and referrals to other providers (as appropriate) outside of operating hours. |

Medication and Durable Medical Equipment |
Prescriptions for medications and durable medical equipment are available and can be filled at community or online pharmacies. Your Care Navigator may provide instructions for obtaining durable medical equipment through online or community medical device retailers. Medications and durable medical equipment are not included in your membership. |

Mental Health Care |
Identification and treatment of clinical diagnoses of emotional and mental health challenges with a goal of enhanced mental wellbeing and transitioning to self-care. |

Musculoskeletal Care (Physical Medicine) |
Use of therapeutic exercise, physical modalities, assistive devices, and patient education to reduce pain, improve mobility and function that is impaired or threatened by disease, injury, or disability through physical therapy and/or chiropractic services, and coordination with community facilities and specialists as appropriate. Free, online fitness classes are also included in our memberships. |

Care Navigation |
At the direction of a Crossover provider, when a member needs treatment that is outside Crossover’s scope of care, Crossover will refer them to local providers, coordinate referrals, and follow up with the member. |
